Managing a parent's diabetes remotely: what to actually track

'Keep an eye on his sugar' isn't a plan. Here are the four specific things worth checking on every report, and why the trend across them matters more than any single reading.

The four things worth tracking on every report are fasting glucose, HbA1c, kidney values (creatinine and eGFR), and the lipid panel — in that order of how often they should change your thinking. Diabetes on its own is manageable with a stable routine; what makes it worth watching closely from abroad is that it quietly affects other systems years before it announces itself, and those effects are the ones a family furthest from the day-to-day is most likely to miss.

Fasting glucose: useful, but only a snapshot

A fasting glucose reading tells you about one morning. It's affected by what was eaten the night before, how well someone slept, even stress — which is why a single high or low fasting number, on its own, usually isn't worth much alarm. Where it's useful is spotting an unusually consistent pattern across several readings taken close together, especially around a medication change.

HbA1c: the number that actually shows the trend

HbA1c reflects average blood sugar over roughly the past three months, which makes it a far more honest picture than any single fasting result. Most labs and clinicians treat a result below 5.7% as normal, 5.7–6.4% as prediabetic range, and 6.5% and above as consistent with diabetes — though the specific target for someone already being treated is usually set individually by their doctor, often a bit higher for an older adult where very tight control can bring its own risks like low-blood-sugar episodes.

What matters most for tracking isn't hitting a specific number — it's the direction across two or three consecutive reports. A parent whose HbA1c has crept from 7.0% to 7.8% to 8.3% over three visits is a different conversation than one who's held steady at 7.5% for two years, even if both are technically "above target."

Kidney values: the connection that's easy to miss

Diabetes is one of the two most common long-term causes of kidney decline in Indian adults — the other being high blood pressure, and the two frequently coexist in the same person. This is the connection that gets missed most often, because the diabetes doctor is watching sugar and the kidney values sit lower on the same report, unremarked on unless something is flagged.

What counts as a normal creatinine and eGFR is worth knowing specifically for this reason: a slow rise in creatinine alongside long-standing diabetes is exactly the pattern worth raising directly with the treating doctor, in those words — "is this connected to the diabetes, and does it change anything about the current medication."

The lipid panel: usually secondary, still worth a glance

Diabetes and cholesterol management often go together, since both affect cardiovascular risk. LDL cholesterol is the value doctors typically manage most actively; triglycerides swing more with diet and are less reliable as a single-reading signal — a spike after a festival week isn't the same as a trend across several fasting reports.

Putting it together

None of these four values needs to be checked in isolation each time — the useful habit is lining them up side by side every time a new report comes in, the same way you would read any of your parent's lab reports from a distance: not "is this one number okay," but "is anything here moving, and is it moving alongside anything else." A rising HbA1c next to a rising creatinine is a more urgent pattern than either alone.

The other habit worth building: bring the actual sequence of numbers to the next follow-up appointment, not a general sense that "the sugar's been a bit high." "HbA1c went from 7.0 to 8.3 over three visits, and creatinine's moved from 1.1 to 1.4 in the same period" gives a doctor something concrete to act on in a visit that often lasts only a few minutes.
